Tom Pidcock may miss part of the Spring Classic campaign after his stomach upset from missing the Strade Bianche recurred in Milan-San Remo.
The British talent was unable to complete Saturday's Monument and was fined by the UCI after his coach, Kurt Bogarts, revealed he had to stop for an emergency bathroom break.
Pidcock fell on the Capi climb before the race got into full swing in Chipressa.
"Tom had to seriously go to the bathroom. Even afterwards, his stomach didn't feel 100% healthy. There was no point in racing any more," Bogerts told Het Laatste Nieuws (open in new tab).
The issue raises further question marks about Pidock's health and threatens to derail his campaign this spring.
The 22-year-old Pidock was not scheduled to run in Friday's E3-Saxo Bank Classic and Sunday's Ghent-Wevelgem, but his run in the Dwars door Hlaanderen, which takes place a week later on Wednesday and just four days before the Tour of Flanders, is also in doubt. The race is also said to be in jeopardy.
Bogaerts noted that Saturday's problems were "similar" to those that caused him to miss the Strade Bianche on March 5, and revealed that Pidcock had undergone blood tests to get to the bottom of the problem. After Strade Bianche, he completed a high-altitude training camp in Andorra and seemed to be back on track, but the problem suddenly returned in Milan-San Remo.
"We will now investigate. Tom will have a blood test on Wednesday and we hope to find the cause based on the results," Bogaerts told HLN.
"His next race will be at Dwars Door Hlaanderen, but that is uncertain right now. Until this issue is resolved, I don't intend to schedule a race."
Pidcock was scheduled to enter the Ardennes Classics from Flanders and finish in Liege-Bastogne-Liege in late April. He was then scheduled to go straight to the Giro d'Italia, but Ineos' management suggested to Cycling News that Pidcock might be switched to his Tour de France debut.
